Definition
The prescribed AI Assessment template organisations must complete at project inception and on major changes to identify and document expected benefits, privacy and safety risks, fairness and bias concerns, testing and validation regimes, and proposed mitigation measures. It is used as a decision-gate artifact to inform governance review and escalation.
