Declaration of Conformity
Written statement by provider that AI system meets regulatory requirements.
Definition
A Declaration of Conformity is a written declaration prepared and signed by the provider of a high-risk AI system stating that the system complies with all applicable requirements of the EU AI Act. This declaration must accompany the AI system and be kept for at least 10 years after the system is placed on the market.
The declaration must include: the name and address of the provider, a statement that the declaration is issued under the provider's sole responsibility, identification of the AI system, a statement of conformity with requirements, references to relevant standards used, and the name and position of the signatory.
The Declaration of Conformity creates legal accountability for the provider and must be made available to market surveillance authorities upon request.
